2024 Tax Facts


Completed tax facts forms must be SUBMITTED by September 15 to
ESA Headquarters via mail or email (no longer accepting faxes).


NEW IRS REQUIRED ADDENDUM:
If you donated $5,000 or more to any ONE charity this year you are required to submit this addendum with your tax facts form.
